Accreditation Canada survey – evaluation of the patient experience

People receiving care and services at the CISSS de l'Outaouais between May 1 and June 30 2022, are invited to complete the Accreditation Canada survey. The purpose of this survey is to evaluate the patient experience in order to improve the care and services offered in one of our facilities.

The survey allows patients to give their opinion regarding :

• The reception
• The cleanliness and safety of the premises
• The respect, courtesy and receptiveness of the doctors and other professionals
• The explanations given concerning the care offered
• The respect of the rights as a patient or resident in one of our facilities

Procedure 

It takes approximately 15 to 20 minutes to complete the survey. A resume code will be provided at the beginning of the survey so it can be completed later, if necessary. The deadline for submitting the completed survey is July 31, 2022.

If necessary, a family member or staff member may assist the patient in completing the survey.

Data privacy

All the survey answers are anonymous and will stay confidential. The database is secured.
