You may have been invited to complete evaluation questionnaires or surveys. These are used to assess the care and services you receive from us, and will help us identify improvement priorities based on your answers.

In the same vein, we encourage you to send us your comments, concerns or suggestions, and to let us know whether or not you were satisfied with your experience with the care and services we provided.
