If you do not have a family doctor, you can register with the Family Doctor Finder (GAMF). This is where you will find the eligibility criteria as well as the instructions on how to register.

Territory waiting list (GACO) and Family Doctor Finder (GAMF)

If you are already registered with the GAMF, there is no need to register again. Your registration and the order of priority assigned to you will be maintained.

If you would like to change the contact information or health information related to your request, please go directly to the GAMF. Your registration will be updated and your waiting priority will be maintained.

If you need guidance, telephone assistance to register or change health information related to your GAMF request, you can contact the territory waiting list (GACO) in your area.

Registration with a Family Doctor

Once you have been assigned a doctor, you must register with them. To do so, you must fill out the registration form they give you at your first appointment.

Registering with a doctor is voluntary and free. However, it may terminate under certain circumstances, such as:

• if you register with another physician;
• if your doctor can no longer provide your medical follow-up (e.g. leaves the clinic, retires), in which case, you can contact your local GACO to request a new doctor: you will keep your current doctor until a new one is assigned to you; or
• if you are moving or unable to travel to your current clinic, you can contact your local GACO to request another doctor: you will keep your current doctor until a new one is assigned to you.

In some cases, you may not be able to request another doctor while keeping your current one. In those cases you must cancel your registration with your current doctor. When you cancel your registration, you may no longer have access to the services of this doctor or the clinic you are attending.

To cancel your registration with your current doctor, you must contact the Régie de l’assurance maladie du Québec (RAMQ). You can then register for the waiting list through the GAMF on the Portail santé mieux-être of the Ministère de la Santé et des Services sociaux or by calling the GACO in your territory.
