Intermediate and family-type resources
These resources are for people with an intellectual disability, an autism spectrum disorder or a physical disability and whose needs are no longer met by their natural environment’s current setup.
The service provides a residential living environment that is as similar as possible to a natural environment, and adapted to the age and needs of people and integrated into their community.
How do I access the services?
To access the program, a letter should be sent to the ID-PD-ASD program access point by a health and social services network social worker.

A patient navigator will assess your needs, develop your response plan, pair you with a resource matching your needs, support you in your integration into the resource and determine the specific support or assistance you need. Most services are delivered within the individual’s living environment:
