Requests for access to the vaccination register
Requests by Outaouais residents are handled by the CLSC Le Guerrier Records Department. The MSSS will continue to process requests from outside Québec.
Procedure for Quebec residents
- Complete and sign the form on the MSSS web site: “Access to information on vaccines request”
- Provide a copy of your health insurance card, or if it is not available, a copy of your birth certificate.
- Send the documents by mail, by fax or by e-mail to:

Procedure for non-Québec residents
- Complete and sign the form on the MSSS web site: “Access to information on vaccines request”
- If the requester does not have a RAMQ number, there’s no need to indicate it.
- Send the form as well as a copy of a valid ID (health insurance card, birth certificate) using one of the following methods:

Did you know that?
For children under 14, a photocopy of a valid ID of the parent, of the representative, or of the legal representative, as well as a piece of the child’s ID (health insurance card, birth certificate) are required.
For their part, individuals aged 14 and older must complete their requests personally.
