Requests by Outaouais residents are handled by the CLSC Le Guerrier Records Department. The MSSS will continue to process requests from outside Québec.

Procedure for Quebec residents

• Complete and sign the form on the MSSS web site: “Access to information on vaccines request”
• Provide a copy of your health insurance card, or if it is not available, a copy of your birth certificate.
• Send the documents by mail, by fax or by e-mail to:

Procedure for non-Québec residents

• If the requester does not have a RAMQ number, there’s no need to indicate it.
• Send the form as well as a copy of a valid ID (health insurance card, birth certificate) using one of the following methods:
